Patient-Centered Medical Home recognition
Saint Luke’s Medical Group–Smithville has been awarded the Patient-Centered Medical Home (PCMH) 2011 Level 3 Recognition—the highest level bestowed by the National Committee for Quality Assurance. Monday – Friday: 8 a.m. – 5 p.m.
Monday – Friday: 8 a.m. – 11 a.m.
Please park in front of the main hospital.
Insurance Plans: We participate with a number of insurance companies. Please review your insurance plan to see if a copayment is required. If so, please be prepared to pay the copayment prior to being seen by the physician. Should you have any balances from previous visits, you will be asked to make those payments at the time of service.
Appointments, cancellations, and arrival times: Please call 816-532-3999 to schedule an appointment with your physician. We ask that if you must cancel your appointment for any reason, you give us 24 hours notice so that we may schedule another patient at this time. Please arrive at least 20 minutes prior to your appointment so that the necessary paperwork can be completed or updated and your copayment collected.
