All our physicians are board certified in either family medicine, a primary care specialty devoted to the diagnosis, management, and treatment of patients from birth through geriatrics, or internal medicine, a primary care specialty devoted to adolescents and adults. Our practice also includes endocrinologists, a specialty that is devoted to the diagnosis, management, and treatment of diseases of the endocrine system, including diabetes, thyroid disease, and related illnesses.

Forms for your visit
Click on the New Patient Forms tab below for the forms to complete prior to your visit. Please print these forms, complete and sign each one, and bring to the receptionist when you check in for your appointment. This will make your check-in process smoother and quicker.

Complimentary valet services are available at the entrance to the Diagnostic Center and medical office building. Parking is located around the medical office building.
We offer a full range of primary care, endocrinology, and psychiatry services including:
- Bone densitometry (Dexa Scan)
- Diabetes education and monitoring
- Thyroid disease treatment and management
- Thyroid cancer
- Growth hormone therapy
Insurance Plans: We participate with a number of insurance companies. Please review your insurance plan to see if a copayment is required. If so, please be prepared to pay the copayment prior to being seen by the physician. Should you have any balances from previous visits, you will be asked to make those payments at the time of service.
Appointments, cancellations, and arrival times: Please call 816-347-5100 to schedule an appointment with your physician. We ask that if you must cancel your appointment for any reason, you give us 24 hours notice so that we may schedule another patient at this time. Please arrive at least 20 minutes prior to your appointment so that the necessary paperwork can be completed or updated and your copayment collected.
