Would I (or someone I care about) benefit from palliative care services? |

If you answer yes to any of the following questions, you might benefit from a palliative care consult. If you (or a loved one) are an inpatient at Saint Luke's Hospital, please discuss this with the physician directing your care or call (816) 932-1353 for more information.
- Are you distressed by physical symptoms (pain, shortness of breath, fatigue, constipation, nausea, loss of appetite, weight loss, difficulty sleeping) relating to a chronic or life-threatening illness?
- Are you distressed by emotional issues (depression, anxiety, spiritual distress) related to coping with a chronic or life-threatening illness?
- Would you like to talk to a team of medical professionals specializing in evaluating and treating symptoms related to a chronic or life-threatening illness?
- Do you have questions about how to talk to your family (or loved one) about your wishes for ongoing medical care? Would you want a team of medical professionals to meet with you and your family to discuss your goals of care and to talk about what to expect in the future from your illness?
- Do you have questions about your prognosis and/or have end-of-life planning concerns?
- Are you suffering from spiritual distress related to your chronic or life-threatening illness?
- Do you have questions about community resources available (home health, hospice, inpatient hospice, in home care, medical equipment, financial assistance, insurance issues, etc.) to assist you and your family during the course of your illness?
