All of our physicians are board certified in either Family Medicine, a primary care specialty devoted to the diagnosis, management, and treatment of patients from birth through geriatrics, or Internal Medicine, a primary care specialty devoted to adolescents and adults.

Forms for your visit:
Click on the "New Patient Forms" tab below for the forms to complete prior to your visit. Please print these forms, complete and sign each one, and bring to the receptionist when you check in for your appointment. This will make your check-in process smoother and quicker.

Parking is provided in the parking lot south of the Saint Luke’s South Medical Office Building. Vans drive around the parking lot and will provide transportation directly to the front door of the Medical Office Building. Free valet parking is also available at the front entrance of the Medical Office Building from 7:30 a.m.–5 p.m.

Relay Health: You may register for this electronic communication service through the Saint Luke’s Health System website or by completing a card at our office. This service allows you to request an appointment, request your lab results, request a prescription refill, talk to your doctor, talk to the office, and, in some cases, have an online visit with your doctor.
Insurance Plans: We participate with a number of insurance companies. Please review your insurance plan to see if a copayment is required. If so, please be prepared to pay the copayment prior to being seen by the physician. Should you have any balances from previous visits, you will be asked to make those payments at the time of service.
Appointments, cancelations, and arrival times: Please call 913-317-7990 to schedule an appointment with your physician. We ask that if you must cancel your appointment for any reason, you give us 24 hours notice so that we may schedule another patient at this time. Please arrive at least 20 minutes prior to your appointment so that the necessary paperwork can be completed or updated and your copayment collected.
